- Number of employees and retirees covered

Pharmacy Reimbursement
- Pharmacy reimbursement for retail, mail and/or specialty pharmacies
Utilization Metrics
- Annual trend rate
- Total number of prescriptions dispensed at retail and mail
- Total number of generic prescriptions dispensed at retail and mail
- Percentage of claim cost paid by member at retail and mail
- Number of scripts Per Member Per Month
- Gross cost of scripts Per Member Per Month
- Net cost of scripts Per Member Per Month
- Annual deductible, maximum annual benefit and out-of-pocket limit amounts
- Rebates amounts per script at retail and/or mail
